本研究目的在探討台灣醫院場域麻醉護理人員工作情境及相對應情境之能力項目的常見性和重要性,並分析不同工作期別(雇用時、起始期、和後續發展期)麻醉護理人員被期待具備的執業能力。本研究採修正型德懷術分兩階段進行,第一階段使用文獻考察與訪談法,描述各類情境中麻醉護理人員應具備的知識、態度、技能與文化等能力,訪談資料採內容分析法進行分析。第二階段將前述所收集的資料分析結果編製為問卷後,邀請28 位麻醉護理德懷專家提供三回合問卷的意見,三回合問卷的回收率均達100%,並將各回合問卷獲得之量性資料進行描述性統計分析(眾數、平均數、標準差以及四分位差),文字意見採歸納法進行項目的增加、刪除與修正。
研究共獲得(一)麻醉護理工作情境八類86 項,常見性及重要性均屬高度者有六類16 項(19%),常見性中度而重要性高度者有八類31項(36%),常見性低度而重要性高度者有七類39 項(45%)。(二)被期待具備的工作能力八類137項,常見性及重要性均屬高度者有八類120項(87%),常見性中度而重要性高度者有二類5項(14%),常見性低度而重要性高度者有四類12項(9%)。進一步分析工作能力歸屬雇用期者有八類119項能力,起始期者有七類15項能力,尚未達共識無法歸類者有一類3項能力。
研究結果可做為設計麻醉護理模組課程之實證性基礎,撰寫麻醉護理工作業務標準、以及作為臨床發展麻醉護理人員訓練課程與評值教學成效之參考。
[ Abstract ]
This study aims to explore the working situations and practicing competencies of anesthetic nurses in Taiwan hospitals, and to examine the importance and frequency of occurrence of these set items. We analyzed the expected practicing competencies of anesthetic nurses in three different working stages(hiring, initiating and ongoing). The entire research was conducted with the modified Delphi Technique in two phases. During the first phase, data regarding competencies in knowledge, attitude, skill and culture were collected by performing methods of deep interview and literatures-review to modify the first-rounder questionnaires of Delphi Technique. Secondly, the data was composed into a questionnaire and 28 senior anesthetic nurses were invited to answer this questionnaire in three rounds. The completion rate for all 3-rounds of the questionnaire was 100%. Quantitative data collected from all three rounds of the questionnaire were then used to conduct descriptive statistical analysis (using means, standard deviation, modes, and interquartile range). Questionnaire descriptions were then modified, added or deleted using the induction method. 
The results indicated that (1) Working situations were composed of 8 categories, including 86 items. The working situations which were graded as highly valued in both importance and occurrence included 6 categories and 16 items (36%), highly important but moderate occurring situations included 8 categories and 31 items (45%), and highly important but low occurring situations included 7 categories and 39 items (45%). (2)The practicing competencies had a total of 8 categories, including 137 items. The practicing competencies which were graded as highly valued in both importance and occurrence included 8 categories and 120 items (87%), the competencies valued highly important but moderate occurring had 2 categories and 5 items (14%), and highly important but low occurring practicing competencies included 4 categories and 12 items (9%). Further analysis of practicing competencies showed that anesthetic nurses in the hiring period had competencies in 8 categories and 119 items, nurses in the initiation period had competencies in 7 categories and 16 items, yet one category and 3 items of practicing competencies are not to be categorized owing to reach less 50% consensus. 
Furthermore, not only could these results facilitate teachers in designing anesthetic nursing modular curriculum, be the foundation to construct working standards, help arrange teaching contents and sequences, and also facilitate learners to choose the curriculum by themselves. In the meanwhile, these valuable data could be offered as the guideline for the development of continuing education and the assessment of training effect.
