| 搶救生命之最前線的119消防局救護技術員,扮演著危急傷病患後續醫療品質及預後的關鍵。由於面對複雜的救護情境與病人疾病,救護技術員常需在職教育來強化他們之救護能力。本研究旨在探討救護技術員在職教育的需求及相關影響因素,包括救護技術員個人背景、課程形式及課程內容等。 本研究採用橫斷式調查法,以現今擔任緊急醫療救護主力之新北市消防局中級救護技術員為對象進行普查,研究工具採用自編式結構性問卷,問卷業經良好的表面效度、專家內容效度及內部一致性檢定。將研究對象個人背景資料、在職教育課程形式需求、在職教育課程內容需求透過描述性統計、t考驗、單因子變異數分析及卡方檢定等統計方法進行資料分析。 研究結果顯示:(一)研究對象最重視在職教育之時間安排,需求最高的教學方法為實務操作,需求最高的課程實施方式為案例分析研討,最喜歡的講師背景為急診科醫師,需求最高的課程內容模組為「病人評估與處置」。(二)在職教育課程形式需求方面,專責救護隊及服務單位月出勤頻率≧300次的救護技術員對教學方法「實務操作」、講師背景之「主題相關專科醫師」需求較高。(三)在職教育課程內容需求方面,專責救護隊或服務單位月出勤頻率≧300次的救護技術員對預立醫療流程、相關法律、高級救護技能及情緒管理等課程需求較高。 希望本研究的結果可貢獻予救護技術員在職教育訓練課程規劃之實證基礎,並作為日後消防機關提升救護技術員在職教育品質之參考。 |

| The Emergency Medical Technician (E.M.T.) standing in the front of rescuing lives in Fire Departments (119) plays the key role in sequential medical care and prognosis for critical patients. Due to face the complicated scenarios and illness, the E.M.T.s always need in-service training to enhance their ability. This study aimed at the study of the need and relative influent factors for E.M.T. in-service training, such as E.M.T. personal background, training modulus, and contents of training course. Our research design adopted cross-sectional survey method, and used overall interviews toward all E.M.T.-2(Emergency Medical Technician – Class 2)who are the main force nowadys in New Taipei City Fire Department as our research basis, and then used the self-constructed structured questionnaire as the study tool. This questionnaire has already been well tested and verified by face, expertise content validity and internal consistency reliability test. Through descriptive statistics - t-test, one-way ANOVA, and Chi-square test to analyze the variation for E.M.T. personal background, training modulus and contents of training course. The study revealled four major findings: Firstly, we found that E.M.T.s regard the time arrangement as the most important part for training course, and the highest Demand for teaching methods are practical operations. Moreover, they prefer case study as their training practice and they like emergency department doctors to be their instructors, and the most requirements for contents of training course are evaluation and treatment for patients. Secondly, for training modulus, E.M.T.s who serve in the ALS (advanced life support) unit or those BLS (basic life support) units which serve frequency is over 300 times per month expressed a significantly higher need for practical operation in the teaching method and specialty doctors for instructors’ backgrounds. Thirdly, in the aspect of contents of training course, E.M.T.s who serve in the ALS unit or those BLS (basic life support) units which serve frequency is over 300 times per month expressed a significantly higher need for advanced directive med ical processes, the relevant laws, ALS skills and emotional management ... etc. Finally, we hope the study results could contribute an evidence-based foundation toward E.M.T. in-service training program, and maybe take it as a reference to improve the quality of the E.M.T. in-service education by fire departments in the future. |
