本研究旨在探討加護病房護理人員經過多媒體互動自學光碟介入,並搭配模擬情境操作練習後,其照護肺動脈導管留置病人之學習成效。研究方法採單組前後測類實驗設計;研究對象為北區區域教學醫院內、外科加護病房護理師,共計44人;研究對象接受兩週之自學光碟介入,並開放一週獨立模擬情境空間及教具練習,於介入措施的前後實施前後測。以SPSS 19.0進行資料建檔與分析,採次數分配、百分率、平均數、標準差、Paired t-test 等方法進行統計分析。研究結果顯示:(一)自學光碟介入後,在肺動脈導管照護知識之後測得分皆顯著高於前測。(二)自學光碟介入後,在肺動脈導管照護態度中,僅「能有助於提升照護肺動脈導管留置病人之自信心」,及「相信自己能提供正確的肺動脈導管臨床數據於決策中」的後測得分顯著高於前測。(三)自學光碟介入後,在肺動脈導管照護技術之後測得分皆顯著高於前測。(四)自學光碟介入後,肺動脈導管照護學習感到滿意及非常滿意者達88.5%。因此,本研究介入之照護肺動脈導管留置病人多媒體互動自學光碟,能提升加護病房護理人員之照護肺動脈導管留置病人之學習成效,是符合護理人員在職訓練需求的教材教法。建議未來研究「多媒體互動自學光碟」之設計須依據訊息處理理論、多媒體學習認知理論、學習者為中心、成人學習之特性來設計;在護理人員在職訓練使用光碟介入的同時,應搭配模擬情境練習操作,以增進知識、技能之學習成效。
[ Abstract ]

This study aims to discuss study effects on caring for hospitalized patients with indwelling pulmonary artery catheter after intervention of disc on interactive self-learning with simulation scene practice by ICU nurses. Method: measurement before and after experiment in single group; Objects: a total of 44 participants including nurses in surgery ICUs of northern regional teaching hospital; Study participants was involved in 2-week self-learning following disc, and participated in practices with teaching aids within 1-week independent simulation scene space. Measurements were performed before and after interventions. SPSS 19.0 was used for establishing files and analysis, and methods such as frequency distribution, percent pa rate, mean value, standard deviation and Paired t-test were used for statistical analysis. Results of study showed: (1) After intervention of self-learning disc, the points measured following learning knowledge of pulmonary artery caring were all significantly higher than before. (2) After intervention of self-learning disc, the points with attitudes of only helping enhance confidence of caring for patients with indwelling pulmonary artery catheter and believing in providing accurate clinical data on pulmonary artery catheter for making decision were significantly higher than before. (3) After intervention of self-learning disc, the points measured after employing pulmonary artery catheter caring technique were significantly higher than before. (4) After intervention of self-learning disc, as high as 88.5% patients felt satisfied, even very satisfied with caring following obtaining knowledge of pulmonary artery catheter caring. Therefore, intervention of multimedia interactive self-learning disc of caring for patients with pulmonary artery catheter can enhance study effects on ICU nurses. It is suggested to research design of interactive self-learning disc based on information processing theory, cognitive theory of multimedia learning, centering on learners and features of adult learning. Simulation scene practice should be added to intervention of disc used on the job by nurses to improve study effects on knowledge and skills.
