體外膜氧合(Extracorporeal membrane oxygenation; ECMO)簡稱葉克膜,為一種心肺輔助裝置,照護葉克膜裝置病患需要多種的技術、認知、和非技術性的如溝通技巧等技能,且需要定期練習與熟悉葉克膜機器,與持續進行在職教育,才能確保照護葉克膜裝置病患是安全。本研究旨為探討加護病房護理人員對葉克膜裝置病患照護在職教育需求與授課方式需求。
      研究以立意取樣,108 位加護病房護理人員為研究對象,採自擬結構式問卷、橫斷性調查法進行資料收集。以次數分配、百分比、平均數、標準差進行描述性統計,以獨立樣本t 檢定、單因子變異數分析、卡方檢定分析、皮爾森積差相關分析等統計方法進行推論性統計。研究結果顯示:護理人員對葉克膜裝置病患照護知識與技能在職教育呈現高度需求,且護理人員年齡愈小,對於葉克膜裝置病
患照護知識在職教育需求程度愈高(r= -.20)。對葉克膜裝置病患照護知識希望授課方式,以「課室教學」需求最多,且達顯著差異(<.00)。對葉克膜裝置病患照護技能希望之授課方式,以「臨床實作」需求最多。
      研究結果建議臨床在職教育安排,應區分為基礎課程與進階課程;醫療單位應添購葉克膜裝置病患模擬器,與葉克膜專家共同擬定培訓課程,定期舉辦認證考試,以確保其照護能力。
[ Abstract ]
Extracorporeal membrane oxygenation (ECMO) is a cardiopulmonary assist device. Caring for paients on ECMO require numerous technical, cognitive and nontechnical skills, such as effective communication skills. Furthermore, regular practice, familiarity with the ECMO machine and continual in-service education are necessary to ensure the safe care of patients on ECMO. This study investigated the needs of intensive care
unit (ICU) nurses regarding in-service education and teaching approaches for the care of patients on ECMO.
Purposive sampling was employed to investigate the needs of 108 ICU nurses. Research data were collected through a self-administered questionnaire and crosssectionalstudy. Frequency distribution, percentage, mean, and standard deviation were used in descriptive statistics, and an independent t test, One-Way ANOVA, a chisquared test, and Pearson Correlation coefficient were adopted for inferential statistics.
The results revealed a high level of need for in-service education regarding the knowledge and skills in caring for patients on ECMO, of which a higher need was found in young nurses (r = ˗.20). The preferred teaching approach for knowledge associated with ECMO care was classroom teaching (P < .00), whereas the majority of nurses preferred to acquire the relevant skills through clinical practice.
The results also revealed that in-service education for clinical staff should be divided into basic and advanced courses and that medical units should purchase simulators of patients on ECMO, collaborate with ECMO experts to develop training courses, and hold regular certification examinations to ensure the care capacity of their
