背景: 成人重症加護中心是個高複雜且具挑戰的環境,然而由於臨床招募人力困難,常使得新進護理人員在不具臨床照護經驗下,便直接進入加護中心工作,嚴重影響病人安全。有鑑於此,發展及評價「成人加護中心新進護理人員重症照護能力指標」,實有其重要及必要性。

目的本研究目的為針對成人加護中心新進護理人員,進行重症照護能力指標之發展並進行調查。

方法:採橫斷式研究設計。本研究共分三階段包括問卷內容初擬、專家德菲法及運用重症照護能力指標問卷,以立意取樣方式,選取北部某區域教學醫院之成人加護中心新進護理人員進行重症能力調查。

結果:「成人加護中心新進護理人員重症照護能力指標」,共有六個構面包含:重症知識能力、重症技術操作能力、團隊合作與溝通能力、維護病人安全能力、緊急應變能力及醫療設備與衛材管理能力。基本屬性的「最高學歷」、「加護中心訓練」及「臨床工作年資」與整體重症照護能力達統計差異(p < .05)。迴歸分析顯示,「專科vs 四技」(β= .34,p < .05)與「二技/學士後vs 四技」(β= .31,p < .05)的迴歸係數達顯著正值,代表當護理人員為專科或二技/學士後畢業時,在重症照護的能力優於四技畢業者;「臨床工作年資」(β= .20,p < .05)的迴歸係數達顯著正值,代表當護理人員的年資越高時,在重症的照護能力也會越佳。

結論:研究結果顯示,「成人加護中心新進護理人員重症照護能力指標」具有良好信度與效度。研究成果可作為評量成人加護中心新進護理人員在重症照護能力的重要工具。

[ Abstract ]

Background: Adult critical care units are highly complex and challenging environments. Difficulty in human resource recruitment can result in new nursing staff working directly in an intensive care unit without having clinical care experience, which can create concerns regarding patient safety. Therefore, development and evaluation of a Clinical Care Competency Index for New Nursing Staff at Adult Intensive Care Units are necessary.

Purpose: In this study, a critical care competency index was developed for application to new nursing staff at intensive care units.

Method: A cross-sectional design was used in this study, which was separated into three stages: (1) preliminary planning of questionnaire content, (2) use of the Delphi method, and (3) implementation of a clinical care competency index questionnaire. A survey on the critical care competency of new nursing staff at an adult intensive care unit of a regional teaching hospital in Northern Taiwan was conducted through purposive sampling.

Results: The resultant Clinical Care Competency Index for New Nursing Staff at Adult Intensive Care Units has six dimensions: (1) knowledge of critical illnesses, (2) technical abilities for critical care, (3) teamwork and communication abilities, (4) ability to keep patients safe, (5) emergency response ability, and (6) ability to manage medical equipment and medical supplies. The relationships of nursing staff’s critical care competency with basic attributes including education level, intensive care unit training, and years of clinical work experience were significant. A regression analysis indicated that nursing staff who had a postsecondary degree, 2-year associate degree, or postgraduate degree exhibited superior critical care competency compared with those with a 4-year associate degree; additionally, years of clinical work experience was positively correlated with critical care competency (β = 0.20, p < .05).

Conclusion: The results suggested that the proposed index had excellent reliability and validity. The research outcome can serve as a crucial tool for evaluating critical care competency of new nursing staff at adult intensive care units.
