隨著醫療技術進步,微創手術已進入達文西機械手臂系統輔助手術時代。達文西機械手臂系統具有3D立體超解析度視覺影像、透過仿人類手腕有7個靈活度的精細器械,提供更為精密、靈巧控制以施行複雜手術。為了面對高科技先進醫療技術,手術室護理人員要特別訓練學習達文西機械手臂系統相關設備、器械、手術技術與知識,使護理人員在達文西機械手臂系統醫護團隊中有自信心並扮演重要角色,讓手術全期照護體系更完善,以提升病人手術安全。本研究目的在探討手術室護理人員對達文西機械手臂系統在職訓練課程內容需求及在職訓練形式需求。
本研究為橫斷式研究設計,採自擬結構性問卷進行資料收集,評估手術室護理人員在職訓練需求。本研究共發放180份問卷有效問卷術為180份,有效問卷回收率為100%。資料使用以次數分配、百分比、平均值及標準差進行描述性統計,以Mann-Whitney U test、獨立樣本t檢定、單因子變異性分析、皮爾森績差相關分析等統計方法進行推論性統計。研究結果顯示手術室護理人員對達文西機械手臂系統在職訓練整體課程呈現高度需求。手術室「已婚」的護理人員在達文西機械手臂課程內容需求的故障排除方法需求較高。在達文西機械手臂系統課程內容需求,以「執行無菌套披覆手術台車」、「執行達文西器械故障時之卸除流程」、「說出達文西機械手臂系統移動及固定操作方式」需求最高。研究結果顯示專科教育程度、已婚、41歲以上、臨床年資16年以上及N2級階以上之護理人員,在情境模擬法、數位學習法、示範教學法、個別化教學法及互動室討論法等有顯著差異,在職訓練形式需求與達文西機械手臂系統課程內容需求有顯著正相關,
研究結果可作為日後課程規劃及在職訓練方式之參考,以增加臨床技術操作熟悉度,並提升護理人員在臨床實務表現及照護品質。
[ Abstract ]
With the advancement of medical technology, minimally invasive surgery has entered the era of assisted surgery in the Da Vinci mechanical arm system. The Da Vinci robotic arm system has 3D stereoscopic super-resolution visual images and 7 flexible instruments through the human-like wrist, providing more precise and dexterous control for complex operations. In order to face high-tech advanced medical technology, the operating room nursing staff should specially train and learn the equipment, instruments, surgical techniques and knowledge of Da Vinci mechanical arm system, so that the nursing staff can have self-confidence and play an important role in the Da Vinci mechanical arm system medical team. The role of the surgery to improve the overall care system to improve patient safety. The purpose of this study was to investigate the need for operating room nursing staff to meet the content requirements and on-the-job training format of the Da Vinci mechanical arm system.
This study is a cross-sectional study design, collecting data from a structured questionnaire to assess the in-service training needs of operating room nurses. A total of 180 valid questionnaires were distributed in this study, and the effective questionnaire recovery rate was 100%. Data were used for descriptive statistics by frequency distribution, percentage, mean and standard deviation. Inferential statistics were performed using Mann-Whitney U test, independent sample t-test, single-factor variability analysis, and Pearson performance-related analysis. The results of the study show that the operating room nurses are highly demanding the overall course of the Da Vinci mechanical arm system in-service training. The “married” nursing staff in the operating room are in high demand for troubleshooting methods in the content requirements of the Da Vinci robotic arm. The demand for the content of the Da Vinci mechanical arm system course is the highest in the "execution of sterile drape surgery trolley", "discharge process when performing Da Vinci equipment failure", "speaking Da Vinci mechanical arm system movement and fixed operation mode" . The results of the study showed that the degree of specialist education, married, 41 years of age, clinical seniority of more than 16 years and N2 level of nursing staff, in the situation simulation method, digital learning method, demonstration teaching method, individualized teaching method and interactive room discussion method There are significant differences, and the need for in-service training has a significant positive correlation with the content requirements of the Da Vinci mechanical arm system course.
The research results can be used as a reference for future curriculum planning and in-service training methods to increase the familiarity of clinical technical operations and improve the clinical performance and care quality of nursing staff.
